Use the ISU Incident Portal to report work-related injuries
The ISU Incident Portal is a comprehensive incident-reporting system for work-related injuries and incidents. (A portion of the system is devoted to non-work-related injuries such as student or visitor injuries, volunteer injuries and others not covered by workers’ compensation.) This system replaces the former injury and incident-reporting system that was available in AccessPlus. It is intended to increase efficiency and automation in incident and injury reporting and tracking by utilizing a single portal with more user-friendly and intuitive interfaces.
The system enables increased collaboration and coordination between University Human Resources, Office of Risk Management, Environmental Health and Safety and others to better serve faculty, staff, students and visitors.
Please note that all faculty and staff need to report work-related injuries using this system. To access the portal, visit UHR’s website or use the alpha index on Iowa State’s homepage (W > Workers’ Compensation > Workers’ Compensation Forms > First Report of Injury).
